After 90 days employment, full time employees are eligible for health insurance coverage. Coverage for Employee Only is $13.50/week thru payroll deduction. Employees may also carry dependent health coverage thru payroll deduction.

Highlights of plan for employees seeing CIGNA Open Access Plus Providers includes $15 Co-pay for Primary Care Physicians, $30 Co-Pay for Specialist, and $300 deductible/80% coverage for other services, with $3,000 maximum out of pocket. Employees not residing within the network area have the same benefits as if care were provided by a network provider.

Pre existing conditions require a one year waiting period or proof of prior insurance.


After 90 days employment, full-time employees are eligible for Pharmacare drug card coverage. The prescription card is available if healt insurance is elected. Most national Chains and major independent pharmacies participate in the Pharmacare network. Employee Co-pays for prescription drugs are $10 for generic, $30, $45, or $65 for name-brand.

After 90 days employment, full time employees are eligible for dental coverage. Depending on the Dental plan elected, coverage for Employee Only is $2.00 or $3.79 per week. Preventive care is covered at either 80% or 100% (depending on the plan chosen) of reasonable and customary charges with no deductible - one visit each six-month period.

Other covered services are paid at 80% or 50% of reasonable and customary charges after a $50 annual deductible. This deductible is per person with a family maximum of $150. Benefits for Major Procedures are excluded for a person until he or she is covered for dental benefits for twelve consecutive months.

The maximum calendar year dental benefit per covered person is $500 or $1,000. Employees may carry dependent dental coverage through payroll deduction. With the enhanced dental plan, you also receive a $1,500 lifetime orthodontics allowance.


After 90 days employment, full time employees are eligible for vision insurance. The basic vision plan allows for one routine eye exam each year with a $15 co-pay and costs the employee nothing (dependents may be added to this plan at no charge).

The enhanced vision plan for Employee Only coverage is $1.79 per week and after a $15.00 co-pay pays 100% of a routine eye exam and either contacts or single vision lense glasses. Some restrictions to apply to both plans.


After 90 days employment, company drivers are provided $35,000 in life insurance. This benefit is provided at no cost to the employee.
